Patent License Agreement

This Patent License Agreement ("Agreement") is entered into on [Date], by and between:

Together referred to as the "Parties."

1. Patent Information

2. Grant of License

7. Intellectual Property Ownership

8. Termination

This Agreement may be terminated by:
