This Model Release (Minor) (the “Release”) is entered into as of [Effective Date] by and among:
Producer/Photographer: [Name/Business Name], with an address at [Address] (“Producer”).

Parent/Legal Guardian: [Guardian Full Name], with an address at [Address] (“Guardian”).
Producer, Guardian, and Minor may be referred to individually as a “Party” and collectively as the “Parties.”
1. Consent and Grant of Rights
1.1 Guardian Consent. Guardian represents they are the parent or legal guardian of the Minor and has authority to sign this Release on the Minor’s behalf.
1.2 Consent to Record. Guardian consents to Producer photographing, filming, and/or recording Minor’s image, likeness, voice, and performance on [Date(s)] at [Location].
1.3 Granted Rights. Guardian grants Producer a worldwide, perpetual, irrevocable (unless stated otherwise), royalty-free right to use, reproduce, publish, display, distribute, transmit, and otherwise exploit the content described below.

1.5 Permitted Uses. Producer may use the content for:

2. Ownership and Approvals
2.1 Producer Ownership. Producer owns the content and all related rights, subject to Minor’s rights under applicable law.
2.2 No Approval Rights. Unless required by law, Guardian and Minor waive any right to inspect or approve finished content, captions, or copy.
3. Compensation
3.1 Compensation. In exchange for the rights granted, Producer will provide:

5. Release of Claims
5.1 Release. Guardian, on behalf of Guardian and Minor, releases Producer and its successors, licensees, and assigns from claims arising out of the authorized use of the content, including claims for invasion of privacy, misappropriation of likeness, and similar claims, to the extent permitted by law.
5.2 No Defamatory Use. Producer agrees not to use the content in a defamatory, misleading, or unlawful way.
6. Guardian Representations
6.1 Authority. Guardian has authority to grant permissions on the Minor’s behalf.
6.2 No Conflicting Rights. Guardian represents no other person’s consent is required.
6.3 Accurate Information. Guardian confirms the information provided is accurate.
7. Revocation (Optional)
