The parties agree as follows:

If payment is not made as agreed, the Massage Therapist may refuse or suspend future services.
5. Cancellations and Missed Appointments
Either party may cancel or reschedule an appointment by giving at least [Number] hours’ notice.
If the Client gives less than the required notice or misses an appointment without notice:

any replacement appointment will be offered only if the Massage Therapist agrees.

6. Client Health Disclosure
The Client confirms that the Client has disclosed any known medical condition, injury, allergy, pregnancy, skin condition, recent surgery, pain issue, or other health concern that may affect massage therapy.
The Client agrees to update the Massage Therapist if any health condition changes before or during the course of services.
7. Consent to Treatment
The Client understands that massage therapy involves manual work on muscles and soft tissues and voluntarily agrees to receive such services.
The Client may ask questions before treatment and may request adjustment of pressure, technique, or body area at any time during the session.
The Client may stop a session at any time.
8. Scope of Services
Massage therapy provided under this Agreement is intended for general wellness, relaxation, and muscle care purposes.
The Massage Therapist does not diagnose illness, prescribe medication, perform spinal manipulation unless legally authorized, or provide medical treatment.
Massage therapy is not a substitute for medical, psychological, or chiropractic care.
9. Professional Boundaries
The parties agree to maintain a professional and respectful relationship at all times.
Only lawful and appropriate massage services within the Massage Therapist’s scope of practice will be provided. Any inappropriate, abusive, threatening, or sexual behavior by the Client is grounds for immediate termination of the session and this Agreement.
10. Draping and Privacy
The Massage Therapist agrees to use appropriate draping and reasonable privacy practices during each session, except as otherwise required by the massage method or applicable professional standards.
Only the area being worked on should be uncovered to the extent reasonably necessary for treatment.
11. Therapist Responsibilities
The Massage Therapist agrees to:

12. Client Responsibilities
The Client agrees to:

follow reasonable aftercare or session-related instructions where appropriate;

behave respectfully and comply with office or service location rules.

13. Late Arrival
If the Client arrives late, the session may be shortened to avoid delaying other appointments, unless the Massage Therapist agrees otherwise.
Unless otherwise stated, the full session fee may still apply.
14. Confidentiality
The Massage Therapist will use reasonable care to keep the Client’s personal and health-related information confidential, except where disclosure is required by law or reasonably necessary for billing, safety, insurance, or professional recordkeeping.
15. No Guarantee of Results
The Client understands that massage results vary from person to person and depend on many factors, including health condition, lifestyle, frequency of treatment, and physical response.
The Massage Therapist does not guarantee pain relief, recovery, or any specific physical or medical outcome.
16. Termination
Either party may terminate this Agreement by giving [Number] days’ written notice.
The Massage Therapist may terminate this Agreement immediately for nonpayment, inappropriate conduct, unsafe conditions, withheld health information, or another material breach of this Agreement.
Any fees earned or unpaid before termination will remain due and payable.
17. Limitation of Liability
To the extent permitted by applicable law, the Massage Therapist will not be liable for indirect, incidental, special, or consequential damages arising from the massage therapy relationship.
The Massage Therapist’s total liability, if any, will be limited to the amount actually paid by the Client for the services directly related to the claim.
18. Governing Law

19. Entire Agreement
This Agreement represents the entire understanding between the parties regarding the massage therapy services and supersedes prior oral or written discussions on the same subject.
